OUR CARE TEAM

One patient. One family. A whole team.

Hospice care is a team effort.

Each patient's care is provided 24/7 by professionals from different disciplines who communicate with one another and work from an individualized plan of care.

WORKING TOGETHER FOR YOU

Care for the whole person.

Depending on a patient's needs, your Legacy Care team may include:

Physician & Medical Director

Provides medical oversight and works with the hospice team and the patient's attending physician to guide the plan of care.

Registered Nurse

Coordinates the patient's clinical care, monitors symptoms, provides education and helps patients and caregivers understand what to expect.

Hospice Aide

Provides personal care and assistance with activities such as bathing, grooming, dressing and other daily needs.

Social Worker

Helps patients and families navigate emotional, practical and social concerns and connects them with helpful resources.

Chaplain / Spiritual Care

Provides spiritual and emotional support based on the patient's and family's individual beliefs, traditions and preferences.

Volunteers

May provide companionship and additional support for patients and caregivers.

Bereavement Support

Provides grief education, resources and support to families before and after the loss of a loved one.

Your family doesn't have to coordinate all of this alone.

That's what the team is for.

A PLACE TO BEGIN

Connect someone with care.

You don’t have to have all the answers. Refer a patient or tell us what’s happening. We’ll listen, explain the options and help coordinate the next step.
