FOR PATIENTS AND FAMILIES

Start with your questions.

Most families don't come to hospice feeling prepared.

Our team will talk with you about what you're seeing, explain how hospice works and help you determine what the next step might be.

You may be wondering…

Is it too soon?

Does my loved one qualify?

Will we have to change doctors?

Can they stay at home?

What happens if their pain gets worse?

Will Medicare pay for this?

What happens in the middle of the night?

These are normal questions, and you don't need to know the answers before you call.

A CONVERSATION CAN HELP

Signs it may be time to ask about hospice.

Every situation is different, but a conversation about hospice may be helpful if you are noticing:

You do not have to wait for a physician to bring up hospice first.

Patients and families can ask questions, request information and begin the conversation themselves.

Changes in day-to-day care.

More frequent hospitalizations or emergency room visits

Increasing pain or symptoms that are difficult to control

Significant decline in strength, mobility or ability to complete daily activities

Increasing need for help from caregivers

Changes in health and priorities.

Changes in eating, sleeping or communication

Progression of a serious or life-limiting illness

Treatments that are no longer working or are creating more burden than benefit

A desire to focus on comfort and quality of life

We help.

If hospice is the right next step, we'll work with the patient's physicians and other healthcare providers to gather the information needed and coordinate care.

We'll also explain what to expect so you aren't left wondering what happens next.
